Survey code request form

Each residential property owner should have received a letter from Leduc County with a unique survey code and instructions for taking the Citizen Satisfaction Survey.

If you didn't receive a survey code, misplaced your survey code or are a renter, submit this form, call 780-979-2391 or email us to request your code.

Your personal information is collected by Leduc County under the authority of section 33(c) of the Alberta Freedom of Information and Protection of Privacy Act. It will be used for the purposes for which it is collected.
